Texas Consumer Rights
The State of Texas requires consumers in Texas to be informed of their rights to obtain health care records as well as to file consumer complaints.
Records Requests
Current and past clients may request a copy of their health care records by reaching Connection Centered Counseling at (720) 727-7373 and submitting a formal records request [Downloadable PDF Records Request].
Client records are stored and maintained in accordance with state law, which requires that Connection Centered Counseling PLLC maintain your records for a period of seven (7) years commencing on the date of termination of services or the date of last contact with the client, whichever is later.
When the client is a child, the records must be maintained for a period of seven years commencing either upon the last day of treatment or when the child reaches 18 years of age, whichever comes later. After this time, your records will be destroyed.
If records can be transmitted digitally and prepared in less than 30 minutes, there will be no charge for records. Record preparation above 30 minutes will be billed to the client at a rate of $175/hour in quarter-hour increments.
Printing and mailing of records will be billed based on the actual cost to produce and mail the records. Fees are subject to change periodically, and you will be notified in advance of any such fee increase.
